Guest Pass:A general registrant may sign up for one guest registration to provide a couresy badge (name
only, no affiliation) for a spouse, partner, family member or other guest to attend one session. This guest
pass provides a name badge. The final program package is not included. Any guest who wants a program packet
and full access to sessions, the reception and meals must register individually and pay the full registration
fee.

Registration Policies:
Deadline: Pre-registration closes on December 5th. Forms and payment must be postmarked no later than
December 5th to be eligible for the pre-registration discount.
Program Participants: Program participants must pre-register by December 5th in order to have their
names listed in the Final Program.
Refunds/Cancellations: Cancellation notice must be made in writing. 90% of registration fees will be refunded if
written cancellation is received before December 5th. Cancellation requests received between December 6th
and January 8th will be assessed a $40.00 service charge. All fees are non-refundable after January 8th.
